New Patient Registration
To register as a patient, we kindly ask that you follow our simple online process. However, if you prefer to register in person, we would appreciate it if you could visit us after 1pm as our mornings tend to be quite busy.
As part of the registration process, we may ask you some personal questions, such as your medical history and contact information, to ensure that we can provide the best possible care for you. Additionally, we may need to ask some questions about your settled status to comply with legal requirements.
We value your privacy and assure you that any information you provide will be treated with the utmost confidentiality. If you have any concerns or questions about the registration process, please do not hesitate to contact us.
How to Register
To register as a new patient at the practice. Please follow the registration process below.
The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.
Check You Live Within The Catchment Area
Enter your postcode to check you live within the practice catchment area.
Complete a New Patient Registration Forms
It is not mandatory to provide proof of identification or your address to register with the practice, however, we would appreciate this information to expedite the registration process as we need to confirm that you live within our catchment area (this information is not stored).
If you cannot complete this form or have any difficulties at all, please contact our Reception Team who will be happy to offer guidance.
Each family member must complete a form separately.
When you have supplied your details you will be asked to make an appointment with one of the nursing team to have a New Patient Check.
Once you have completed the form, we will be in touch to arrange a New Patient Check.

If you are already registered with us and move out of the area, we may still be able to provide you with medical services. Please complete a change of contact details form or contact reception.
Complete a Carer Registration Form
It is important that we know if you are a carer so that we can make sure you receive information, services and the help that is available.
New Patient Check
You are not fully registered until you have attended your new patient appointment as it helps us to assess your ongoing needs.
When you have supplied your details you will be asked to make an appointment with one of the nursing team to have a New Patient Check.

Medical Card
If you are new to the NHS, your letter of confirmation will take between 2 and 3 weeks to arrive from the Health Authority.
Temporary Services
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Your Named GP
All Patients registered with the practice will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the practice. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
